DESCRIPTION: Reports to CNO, 12 FTE’s The Director Case Management & Social Services position is responsible for strategy, function and operations of the Case Management & Social Work department.
Position is accountable for employee engagement, adequate staffing levels, budget development and compliance, staffing decisions such as hiring and terminating employment, coaching and counseling employees on work related performance.
Ensures a safe and effective working environment; monitors and/or revises the department safety plan and/or any specific accreditation/regulatory required safety guidelines.
Responsible for staff maintenance of credentials and competencies, per accrediting/ licensing agency and/or department guidelines as applicable.
Partners with physicians, quality, palliative care and other departments to identify strategies to improve the mortality index and readmission of patients.
Develops and manages department operational and capital budgets, approvals, and ongoing maintenance of the department(s), ensuring operation in a cost-effective manner.
EDUCATION AND EXPERIENCE:
Texas or Compact state RN REQUIRED
Master's degree (have until December 2022 to obtain)
Five years of management experience in a healthcare environment, three years management experience
Social Worker (LCSW or LMSW) license in State of Texas Preferred
Certification approved by National Association of Social Workers (NASW), or Commission for Case Management (CCM), American Case Management Association (ACMA), or National Board for Case Management
